Step 1) Create an account here

  • Sign up for all sports Sign-ups.
  • Submit or update medical clearance.

Step 2) Complete this  FORM and present to the medical staff conducting your physical examination (please note some facilities may already have a form)

Step 3) Wait for your confirmation email as an indication your athletic clearance has been approved.
